How To Identify If A Form Is Anonymous
Whether you’re filling out a poll on your political leanings or simply answering what cookie you like best, it’s important to be able to identify if the creator of a form can see your name when looking at your response.

Microsoft Forms has four combinations they offer to form creators for recording responses. The form owner can chose between recording responses completely anonymous (allows multiple responses per person), requiring sign-in (allows to limit to an organization, but still allows multiple responses per person), limit to one response (limits to one response, requires sign-in), and record name (records name and emails of respondents, allows limiting to one response, requires sign in).
In simpler terms, your information will only be visible to the form creator if the following line is present under the title of the form.
